I believe in God, the Father Almighty, Creator of heaven and earth.
I believe in Jesus Christ, His only Son, our Lord.
He was conceived by the power of the Holy Spirit and born of the Virgin Mary.
He suffered under Pontius Pilate, was crucified, died, and was buried.
He descended to the dead.
On the third day He rose again.
He ascended into heaven and is seated at the right hand of the Father.
He will come again to judge the living and the dead.
I believe in the Holy Spirit, the holy Christian* Church, the communion of saints, the forgiveness of sins, the resurrection of the body, and the life everlasting. Amen.

*Christian: the ancient text reads “catholic,” meaning the whole Church as it confesses the wholeness of Christian doctrine.

I am willing to protect the confidentiality of all individuals who have or may request service from Love INC of Lake County. I agree that I will not disclose or discuss information regarding any client (neighbor), or source of verification, to any unauthorized person or persons outside my sphere of service (prayer warriors, office, phone volunteer, contact person or other).

We believe God loves all people equally. Love INC of Lake County will serve anyone in need. "But God demonstrates his own love for us in this: While we were still sinners, Christ died for us." Romans 5:8.

I understand and authorize that my image may be photographed or filmed and used in video presentations, printed publications, and/or internet websites associated with Love INC of Lake County.

We collect and use your personal information in accordance with our privacy policy. By providing your contact information, you agree that we may call, email, or text you regarding volunteer opportunities or other Love INC ministry activities.

I acknowledge that my participation in Love In the Name of Christ of Lake County is voluntary and may include involvement in activities that require traveling or physical exertion. I recognize that some ministry projects bear the possibility of unforeseen accidents, health hazards, and medical emergencies that may result in property damage, bodily injury, or death; therefore, in consideration of my willingness to volunteer at Love INC of Lake County, I agree to the following:

Love INC of Lake County is not responsible for the loss or theft of personal belongings.

Love INC of Lake County cannot be held liable for my well-being or safety.

I hereby take the following action for myself, my executors, administrators, heir, next of kin, successors and assigns:

1. I waive, release, and discharge from any and all claims or liabilities for death or personal injury damages of any kind, which arise out of or relate to my volunteering at Love INC of Lake County the following persons or entities: Love In the Name of Christ, Love In the Name of Christ of Lake County, all churches partnering in any capacity with Love INC of Lake County, and any and all paid staff members, Board members and volunteers associated with any of the above entities; and any other representatives, subcontractors and agents of any of the above.
2. I agree not to sue any of the persons or entities mentioned above for any of the claims or liabilities that I have waived, released, or discharged herein.
3. In the event of gross negligence on the part of any of the above-named entities and/or individuals, I agree to settle any dispute by means of Christian arbitration.
4. I indemnify and hold harmless the persons or entities mentioned above from any claims made or liabilities assessed against them as a result of my actions. I hereby assume the risks of my participation in all activities associated with Love INC of Lake County.
